November 19, 2025

TriStar Centennial Medical Center has earned an “A” Hospital Safety Grade for the 14th consecutive time by The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety. This achievement places TriStar Centennial among an elite group of hospitals designated as “Straight A” facilities, those that have maintained an “A” grade for at least five consecutive grading cycles. Fewer than 12.7% of hospitals nationwide hold this distinction.

“Safety and quality are central to our mission at TriStar Centennial,” said Tom Ozburn, chief executive officer at TriStar Centennial. “Earning the straight ‘A’ designation for the past 14 years clearly demonstrates the dedication of every colleague, from those maintaining our facilities to caregivers at the bedside. I’m proud of our team’s commitment to excellence and to serving our patients and our community.”

The Leapfrog Hospital Safety Grade uses up to 30 national performance measures from the Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey and information from other supplemental data sources. Those measures produce a single letter grade twice a year representing a hospital’s overall performance in keeping patients safe from preventable harm and medical errors.
