Sixth consecutive "A" for patient safety earned TriStar Stonecrest Medical Center
Smyrna, Tenn. — TriStar StoneCrest Medical Center, part of HCA Healthcare, has earned an “A” Hospital Safety Grade from The Leapfrog Group for spring 2026, marking the sixth consecutive grading cycle the hospital has received the highest score for patient safety.
With this latest recognition, TriStar StoneCrest has also achieved the elite “Straight A” designation, an honor given to hospitals that have earned an “A” in at least five consecutive grading cycles. Only 15% of hospitals nationwide received this distinction in the spring 2026 grading cycle.

The Leapfrog Hospital Safety Grade is the only hospital ratings program focused exclusively on preventable medical errors, infections and injuries that can harm patients. The grade is assigned twice a year and is based on up to 30 national performance measures from the Centers for Medicare & Medicaid Services, the Leapfrog Hospital Survey and other supplemental data sources.
